Soft Focus is a much sought out effect in photography. You can find it used commonly in glamour shots and to some extent in wedding photography as well. There are special lenses you can use to create the soft focus effect using your cameras. Some modern cameras come with a settings that let you create soft focus effects as well. In any case if any of the above options are unavailable you can easily create soft focus effect in Photoshop
If you own a SLR camera, you can crank your aperture all the way down (the lower the number 1.4/2.8/3.5, the bigger the lens hole gets and the less focused your background gets).
Another way is to purchase http://www.lensbaby.com/#0, its a special kind of lens that lets you "tilt" focus and create beautiful blurs!

Please, please don't put Vaseline on your lens.
The other suggestions are spot-on - a little bit of Vaseline on a filter, pantyhose stretched across your lens and secured tightly with a rubber band, and one friend of mine used crinkled cellophane (like the kind used for cigarette package wrappers) over the lens.
