You should read what you can find on the Web and talk to your homeowner policy agent, many times you can get a rider on your policy for cheap.

Just about any insurance company can provide insurance. State Farm, Allstate, etc.. all have business liability and equipment insurance.
You can probably get a better deal if you go through a professional photography organization like PPA, WPPI, or ASMP.
Some companies like Pictage will have deals for insurance for members.
You should expect to pay between $500 to $1,500 per year depending on the amount of coverage, your credit score, and your time in business.
Make sure you obtain at least $2 million in liability. That is the minimum required by most companies if you ever want to work with them. For example, if you want to do a photo shoot at a hotel, the hotel will want proof that you have at least $2 million in coverage before they will give you permission to shoot there.
